COVID-19 TESTING
If wanting to be a new patient in our clinic, please click on our link "new patient registration" and fill out the forms before requesting an appointment. This can be done on a computer, laptop, or your phone.
After forms have been completed, please call and speak to our front desk to schedule an appointment. We also have paper forms that can be filled out at our clinic if you do not have online access. We also offer same day appointments (provided that availability exists).

If you can't access the registration site for any reason, give us a call and we can send you the packet
After forms have been completed, please give us 48 hours to process and then call and ask to speak to Melanie to schedule an appointment.
